AB OBJECTIVE: Multi-colour flow cytometry has demonstrated significant
alterations within the CD8 subsets co-expressing activation markers in
HIV infection. Our study focuses on the correlation of these CD8 subsets
with viral load in asymptomatic HIV infected patients with no marked CD4
lymphocyte depletion. PATIENTS AND METHODS: Blood samples were obtained
from 49 HIV infected patients and 23 HIV- individuals. All HIV infected
patients were asymptomatic, were accrued within two years of diagnosis
(14.4 +/- 1.3 months), and had a CD4 cell count above 400
cells/microliters (788 +/- 43). An extensive panel of three colour
reagent combinations was assessed by flow cytometry. Levels of HIV
viremia were measured both in PBMC's and in plasma using end point
dilution cultures. RESULTS: Thirty-nine patients had a positive cellular
viremia (0.5 to 5315 TCID50/10(6) cells), while only 5 had a positive
plasma viremia. Mean CD8% was 50.9 +/- 1.5 in patients versus 33.3 +/-
1.3 in controls. Bright CD8 accounted for the expansion of the CD8
cells. All subset results are in proportion of the bright CD8

ABSTRACT VOLUME. CONCLUSIONS: We confirm and extend the finding of
increased specific CD8 subsets to asymptomatic HIV infected individuals
with no marked CD4 depletion. Among these subsets, %CD38+CD45RO+ and
%CD38+CD28- are significantly correlated to cellular viremia. HIV
plasmatic RNA quantification is in progress. A longitudinal study will
determine if these subsets can be used as a prognostic indicator.
